Enhanced Care Tier 1
- 24 hour Nurse Visit-May include, but not limited to, assessment/monitoring of changes in health status, emergency care as needed.
- Medication management and administration of oral medications oral medications
- Supervision and cueing with bathing, dressing, grooming, toileting, ambulation. Reminders and encouragement to attend activities and escort to meals.
- Personal Laundry-includes washing, drying, folding, and putting clothing away. Family to provide clothes hamper.
- Light housekeeping-garbage removal, beds made, linens changed, room order.
Enhanced Care Tier 2
- 24 hour Nurse Visit-May include, but not limited to, assessment/monitoring of changes in health status, emergency care as needed.
- Medication management and administration including but not limited to of oral medications oral medications, topical creams, eye drops.
- Minimal to moderate assist with bathing, dressing, grooming, toileting, ambulation. Reminders and encouragement to attend activities and escort to meals.
- Personal Laundry-includes washing, drying, folding, and putting clothing away. Family to provide clothes hamper.
- Light housekeeping-garbage removal, beds made, linens changed, room order.
Enhanced Care Tier 3
- 24 hour Nurse Visit-May include, but not limited to, assessment/monitoring of changes in health status, emergency care as needed.
- Medication management and administration including but not limited to of oral medications oral medications, topical creams, eye drops, diabetes monitoring to include but not limited to injections, frequent labs for INR management and or other medication management related to routine labs.
- Maximum assist with bathing, dressing,grooming,toileting,ambulation. May use mechanical lift for transfers and mobility. Specialized diet modification and assistance/encourgament with feeding at meals. Reminders and encouragement to attend activities and escort to meals.
- Personal Laundry-includes washing, drying, folding, and putting clothing away. Family to provide clothes hamper.
- Light housekeeping-garbage removal, beds made, linens changed, room order.
Enhanced Care Tier 4
- 24 hour Nurse Visit-May include, but not limited to, assessment/monitoring of changes in health status, emergency care as needed.
- Medication management and administration including but not limited to of oral medications oral medications, topical creams, eye drops, diabetes monitoring to include but not limited to injections, frequent labs for INR management and or other medication management related to routine labs.CPAP, neb treatments, oxygen, wound-care, specialized treatments as ordered per Physician.
- Total care with bathing, dressing, grooming, toileting, ambulation. May use mechanical lift for transfers and mobility. Specialized diet modification and fed all meals. Reminders and encouragement to attend activities and escort to meals.
- Personal Laundry-includes washing, drying, folding, and putting clothing away. Family to provide clothes hamper.
- Light housekeeping-garbage removal, beds made, linens changed, room order.
